Welcome to the Ambetter from Magnolia Health page.  The documents below have been designed to help RadMD users navigate the prior authorization process for each program Evolent is responsible for. PROGRAM UPDATE for Ambetter from Magnolia Health - Exchange: Evolent is no longer managing authorization requests for Pacemaker Insertion, Cardiac Resynchronization Therapy (CRT), Implantable Cardioverter Defibrillator (ICD) and Heart Catheterizations under the Advanced Imaging program in RadMD. Effective January 9, 2026, all authorization requests for these services and members must be submitted through Evolent's CarePro provider portal at www.evolent.com/provider-portal or by calling 1.888.999.7713, option 1. 

ICHRA members can easily be recognized by the "Solutions" section on the member ID card as seen below.
