Welcome to the Trillium Health Resources page. The documents below have been designed to help RadMD users navigate the prior authorization process for each program Evolent is responsible for. New Programs: Effective April 1, 2026, select MSK surgeries and IPM procedures will require prior authorization through Evolent prior to being rendered in a provider office, outpatient hospital, inpatient (MSK only) or ambulatory surgery center for Trillium Health Resources Tailored Plan Medicaid members. Additional information and scope details are outlined under each program.
Advanced Imaging Program
Prior authorization includes the following outpatient procedures: CT/CTA, MRI/MRA, PET Scan, MUGA Scan, Nuclear Stress Test, and Echocardiography.
