Partners Health Management

Welcome to the Partners Health Management page.  The documents below have been designed to help RadMD users navigate the prior authorization process for each program Evolent is responsible for managing. Additional process-specific training tools and modules can be found via links on the Resources tab at the top of the screen.

Alert: Please be aware that effective 10/1/2026, Evolent will no longer be responsible for processing Partners Health Management member authorization requests. This portal will no longer accept authorization requests for these members at that time. For assistance during this transition, providers should contact the health plan. Authorizations currently in place through Evolent for Partners Health Management members will remain valid until the authorization expiration date. Please contact Partners Health Management for new authorization requests with a treatment start date on and after October 1, 2026.

New Programs: Effective April 1, 2026, select MSK surgeries and IPM procedures will require prior authorization through Evolent prior to being rendered in a provider office, outpatient hospital, inpatient (MSK only) or ambulatory surgery center for Partners Health Management Tailored Plan Medicaid members. Additional information and scope details are outlined under each program.
