Interventional Cardiovascular Program-Effective August 1, 2026

Effective August 1, 2026, select interventional cardiovascular services will require prior authorization, through Evolent, prior to being rendered in outpatient or inpatient settings (planned professional services only). The Evolent interventional cardiovascular program will apply to all ordering provider specialties for the following non-emergent elective procedures for Louisiana Healthcare Connections Medicaid members 21 years of age and older:

  • Cardiac catheterization and intervention 
  • Electrophysiology
  • Vascular radiology and intervention 
  • Cardiac surgery
  • Vascular surgery
