Welcome to the ConnectiCare Non-Core (Commercial) page.  The documents below have been designed to help RadMD users navigate the prior authorization process for each program Evolent is responsible for managing. Additional process-specific training tools and modules can be found via links on the Resources tab at the top of the screen.

  • Program Update for Radiation Oncology:  Effective January 1, 2026 please obtain radiation oncology authorizations through the Evolent CarePro Provider Portal evolent.com/provider-portal to submit authorizations for ConnectiCare's Medicare and Exchange Membership. The Non-Core (Commercial) membership is excluded, and you will continue to submit authorization requests through the Evolent RadMD Portal.
