Welcome to the ConnectiCare page.  The documents below have been designed to help RadMD users navigate the prior authorization process for each program Evolent is responsible for. Effective January 1, 2026, the ConnectiCare radiation oncology program will move to the Evolent CarePro Provider Portal evolent.com/provider-portal for ConnectiCare's Medicare and Exchange Membership. The Non-Core (Commercial) membership is excluded, and you will continue to submit radiation oncology requestions through the Evolent RadMD Portal.
